It’s a mechanical process done with the lens. So, when you’re taking pictures, your lens is parallel to the sensor right? This means that your focus plane (the area which is in focus) is also parallel to your sensor/film. But from time to time you may not want that to be the case. Maybe you’re taking a picture of two mountains, and one is behind the other (from your perspective). To get them both focused, you “tilt” the lens in such a way that the focus plane is tilted in such a way that both mountains fall inside the focused area. This is tilting, which changes the relationship between your sensor and the focus plane. Shifting is for other, more complex uses (ever wondered why you can’t take pictures of buildings with straight vertical lines like they have in professional photos? that’s because they’re “shifting”, which is moving the lens up/down, left/right while keeping the focus plane parallel to the sensor/film). For those very big wooden cameras you can just move the lens around because their body is not completely solid. But in solic cameras like SLRs, you need a lens especially made for it. They’re tilt/shift lenses.

So, to get that miniature effect. So, if you want to take a picture of the ground and have it all in focus, you would have to tilt the lens towards the ground, right (this is actually not correct because of complicated optics and geometrics but i’d need to draw to explain it, just go with it)? To make it as close to parallel to the ground as possible. Miniature effect is achieved by doing the complete opposite. Basically making it so the focus plane is perpendicular (almost/more than, not sure) to the ground. Then you speed up the video with some frameskipping and it looks like they’re moving in stop motion. I’m pretty sure you can do it in software too, but that’s super lame.
